To create a Google Form for this survey, follow these steps:
- Go to Google Forms (forms.google.com) and sign in to your Google account.
- Click on the “+” button to create a new form.
- Enter a title for the form, such as “Evaluation of High School Students’ Performance, Effort, Achievements, Skills, and Goals for English Language Arts Course - Quarter 1.”
- Enter a description for the form, such as “Thank you for taking the time to complete this survey. Your feedback will help us understand your performance, effort, achievements, skills, and goals for the English Language Arts course during Quarter 1. Please answer the following questions honestly and to the best of your ability.”
- Click on the first question field and select “Short answer” as the question type.
- Enter the question “Name:” as the question title.
- Repeat steps 5-6 for the remaining questions, using the provided questions in the survey.
- For the performance and effort rating questions, change the question type to “Multiple choice” and enter the rating scale options as the answer choices.
- For the achievements and goals questions, change the question type to “Multiple choice” and enter the options as the answer choices.
- For the skills rating questions, change the question type to “Multiple choice” and enter the rating scale options as the answer choices.
- Click on the “Required” toggle switch for each question to make them mandatory.
- Click on the “Responses” tab at the top of the form editor.
- Click on the “Accepting responses” toggle switch to start accepting responses.
- (Optional) Customize the theme and design of the form using the options in the “Theme” tab.
- Click on the “Send” button at the top-right corner of the form editor to get the link to share the form or embed it on a website.
Your Google Form for the survey should now be ready to use.
